  • Can provide clinically necessary treatment to DVA clients upon referral from treating Doctor or hospital discharge planner. Referral needs to include the condition to be treated.*

 

 

EPC / CDM PROGRAM

(If eligible, Medicare will rebate up to 5 sessions per year at $58.30 per session, speak to your GP to see if you are eligible. Full upfront fee below still applies. Assistance with Medicare online claiming available with additional $10 administration fee.)*

CANCELLATION POLICY

  • Cancellations made within 24 hours will incur the charge of the full treatment (this includes cancellations on arrival).

  • Exceptions may be made if the patient was sent to hospital within this 24hr period.

  • If patient has a diagnosis of cognitive impairment, then the responsibility to arrange appointments shall be passed on to the nominated person/guardian, in which, the above two points apply.

PAYMENT TERMS AND CONDITIONS

These payment terms and conditions ("Terms") outline the payment obligations between My Physiotherapy Pty Ltd ("the Company") and its clients ("Clients"). By engaging in services provided by the Company, Clients agree to adhere to these Terms.

​

1. Payment Obligation: Payment for services is required in full prior to the scheduled treatment session. Clients are required to settle all outstanding balances before the commencement of any services.

​

2. Accepted Payment Methods: Clients may settle their invoices using the payment methods outlined on the Company's official website or provided on the payment link. Accepted payment methods typically include but are not limited to credit/debit cards, bank transfers, or other electronic payment methods. Cash arrangements must be made and agreed upon prior to the scheduled treatment session.

3. Late Payments: In the event of late payment, the Company reserves the right to charge interest on the outstanding balance at a rate of 10% per month or the maximum rate permitted by law, whichever is lower. Additionally, the Company may suspend services until all outstanding payments are settled.

​

4. Legal Action: Failure to comply with the payment terms outlined herein may result in legal action to recover outstanding debts. Clients are liable for any costs incurred by the Company in pursuing such actions, including but not limited to legal fees and court expenses.

5. Dispute Resolution: Any disputes regarding payments or invoices should be raised promptly with the Company's accounts department for resolution. Both parties agree to make reasonable efforts to resolve any disputes amicably before escalating the matter further.

​

6. Modification of Terms: The Company reserves the right to modify these payment terms and conditions at any time. Clients will be notified of any changes, and continued use of the Company's services constitutes acceptance of the updated Terms.

7. Governing Law: These Terms shall be governed by and construed in accordance with the laws of the jurisdiction of where the session took place. Any disputes arising out of or in connection with these Terms shall be subject to the exclusive jurisdiction of the courts in the jurisdiction of where the session took place.

​

By engaging in services provided by the Company, Clients acknowledge that they have read, understood, and agreed to these payment terms and conditions.
